Reliance Care, LLC is a Minnesota DHS-licensed, minority-owned Home and Community-Based Services provider based in the Twin Cities, proudly serving clients statewide. Alongside waiver-funded care, we also work with individuals and families who are arranging services through private pay.

Individualized In-Home Support
One-on-one support at home for daily living, health and wellness, household management, and community participation — customized around each person's goals, schedule, and level of independence.
Community Living Support
A bundled package of daily living, household, health and wellness, and community engagement support — designed to help individuals remain safely and comfortably in their own home.
Homemaker & Companion Services
Hands-on help with cooking, light housekeeping, laundry, errands, and daily routines, paired with friendly companionship — so the home stays clean, safe, and comfortable to live in.
Respite Care
Short-term, in-home or out-of-home relief care that gives family caregivers time to rest, travel, or attend to other responsibilities, with peace of mind that their loved one is well cared for.
Independent Living Skills Coaching
Individualized coaching in budgeting, cooking, transportation, communication, and other everyday skills that build lasting confidence and independence.
Employment Services
Person-centered job coaching — including resume support, interview preparation, and on-the-job guidance — to help individuals find, prepare for, and succeed in meaningful employment.
Additional Support
24-Hour Emergency Assistance, Adult Companion Services, and Night Supervision are also available to round out a private pay care plan.
